MAP to health - Prevention Programme

Within Bexley we are currently piloting a prevention programme for those patients identified by their practice as being at risk of developing type 2 diabetes.  The programme will be piloted at four practices within Bexley and they will invite their own patients along to attend the one 3.5 hour session on prevention.  After the pilot we will evaluate and decide whether to roll out the course to other practices in Bexley.

The programme itself was developed by the Bexley Care Trust team and therefore is new and never seen before.  The programme title MAP to health, means My Action Plan to health, the idea behind the programme is to invite patients to plan their own route out of pre-diabetes. 

The practices set to run the pilots are Ingleton Avenue Surgery and Bellegrove Road Surgery in Welling.  Plas Meddyg Surgery in Bexley village and The Albion Surgery in Bexleyheath.  The pilot sessions will be running over the next few months and we will then evaluate their success.
