Diabetes UK Annual Professional Congress - March 2012
The Bexley diabetes team will have have 3 posters published at the Diabetes UK Annual Professional Congress, in March 2012. This annual conference brings together some of the most important figures within the world of diabetes and allows them to share their successes and findings with the larger diabetes community. Last year the conference was held at London's Excel centre and looked at meeting future challenges in diabetes. Our team will also be showcased as part of the QIC award scheme.
This years conference is being held in Glasgow and will focus on overcoming hurdles and acheiving success. A number of health care professionals from the Bexley diabetes team will be attending the congress and explaining what we have done to improve the care people receive in Bexley.

Diabetes in Primary Care - December 2011
The Bexley diabetes service featured in an article in the December edition of the journal Diabetes in Primary Care. The article written by programme manager John Grumitt and local GP Dr. William Cotter explored the redesigning of the diabetes service in Bexley. The process involved working with people with diabetes and healthcare professionals, improving access to structured education, regular auditing and implementing patient-centred care, including care-planning. Bexley was also a Year of Care pilot site and changes to the service have resulted in improvements in HbA1c levels, blood pressure and BMI, and a sense of empowerment among people with diabetes to self-manage their condition successfully. This article outlines the process of redesigning the diabetes service.

World Diabetes Congress - Dubai - December 2011
Programme manager John Grumitt attended the world diabetes congress in Dubai in December 2011. This event brings together all of the world leaders in diabetes care and research. The event organised by the International Diabetes Federation takes place every year to explore any new findings and breakthroughs in the world of diabetes. At this world congress John, Bexley Diabetes Programme manager, was elected as Vice President of the International Diabetes Federation.
Presentation at Westminster Health Forum - December 2011
Anne Goodchild lead diabetes specialist nurse for Bexley Diabetes team presented at the Westminster health forum on the 7th December 2011. The forum brought together a number of key diabetes figures from around the UK and aimed to discuss "Diabetes - innovation, fostering quality and tackling variations in care." Anne presented what we have acheived with the diabetes services in Bexley.

Lakeside Medical Practice wins QIC award - November 2011
Lakeside Medical Practice of Yarnton Way, Thamesmead, won a Quality in Care award for their Diabetes team. The award for NHS team of the year working in diabetes, was awarded to Lakeside Medical Practice in November last year. The team includes GP lead, nurse practitioner, health care assistant and administrative support. They displayed a great working environment with positive effects; including referrals to X-Pert structured patient education.

Bexley Diabetes team nominated for HSJ award - November 2011
The Bexley Diabetes team were nominated for a HSJ award and made it to the shortlist. These awards have been rewarding innovative changes to health services for 30 years. The Bexley diabetes team received their nomination for the services provided for people with long term conditions.

Better health outcomes: Good practice case studies - Autumn 2011
In the Autumn of 2011 the Bexley Diabetes service featured as a good practice case study within the Primary Care Commissioning, Better Health Outcomes publication. Social finance: Social Impact Bonds a new way to invest in better healthcare - September 2011
Prevention is not only better healthcare but is better economics. This report by Professor Paul Corrigan discusses the potential use of Social Impact Bonds (SIB) in the health sector. How good networks help to implement care planning? - March 2011
Bexley diabetes service holds a monthly network meeting which brings together health care professionals from all areas of diabetes care as well as local patient representatives.
